Location: Marin County, California | Type: Full-time / Part-timeApply NowJob Description SummaryThe registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Key ResponsibilitiesPerform initial patient and family assessments, including physical exams and medical history.Develop and update care plans with preventive, therapeutic, and rehabilitative actions.Administer medications and treatments as prescribed.Provide patient/family education and counseling.Facilitate discharge planning and act as Case Manager when assigned.Collaborate effectively with physicians and community health resources.Supervise home health aides—evaluate them every two weeks.Manage on-call rotation, and coordinate equipment, supplies, and related services.QualificationsGraduate of accredited nursing program with 1–2 years acute care experience.Active RN license in State of California and current CPR certification.Bachelor’s degree and prior home health experience preferred.Excellent communication, observation, and problem-solving skills.Physically able to lift, transfer, and position patients.Valid driver’s license and reliable, insured vehicle.Salary Range: $100,000 to $130,000 Plus Benefits
